This just doesn't seem that crazy to me. That area sticks out a little more than that area around it. It just got snagged.

It's very easy to get injured around heavy machinery. I know, because I work around heavy machinery. Everybody who's been there more than a year been to the hospital atleast once. No manhood manglings yet, but its definately possible.

There are such procedures in place. Things like lock out/tag out before opening gear boxes, removing hand cranks before starting the machine, keeping out of the movable range of the machine while its running.

90% of the time when someone injures their self on the job it's their own fault. I doubt this was any exception. But not taking the proper safety precautions with a machine is completely different than trying to copulate with it.
