I need to save a file as a 4-bit bitmap in Photoshop, but after reducing it to 16 colours, the only available option is 8-bit. How can I save as 4-bit?

Use ImageMagick, that always works. Particularly, the use of the -depth option on convert.

copy-paste final image into MS paint, save as 16 color bitmap, its default (and only option) is 4 bit depth.

Thing is that Paint dosen't save the image with a custom pallete (which photoshop does) which usulaly result in ugly colors.

Yeah, I tried Paint and it was uuuuuuugly.

[edit] ImageMagick created an identical bitmap almost half the filesize, but how can I tell if it's 4-bit? Looking in a hex editor doesn't help much. Photoshop shows it as indexed colour (which could be anything up to 8 bits), but Explorer's properties dialog says it's 32-bit, which is obviously wrong.

I think the fact that it is half the size would tell you it's now in 4bpp. Well, it shouldn't be exactly half the size since the file contains things like BITMAPINFOHEADER and BITMAPFILEHEADER. But if the bmp had large dimensions it should be rougly half at this point.

Try IrfanView. It tells you how much colors the image has, etc. I think you can also save it as a 4-bit + the colo reduction is kinda good (I use it for my hack. )

Okay that's odd... I just tried ImageMagick myself, and the -depth option isn't working as expected

Can you install the GIMP? Just reduce the image to have an index of 16 colors, save as Microsoft BMP (simply use the .bmp suffix to have it auto-selected ), and that works (seriously, I tried it for myself this time)
