The only non-standard cursor I would even deem borderline acceptable is the help cursor, for things that well, help and/or inform you. Like mouseover definitions... maybe. And that's pushing it... |

Custom cursors on websites are evil. They probably rank up there with popups, flash menu navigations, anything that uses Java, music, marquee tags, in the top 10 most annoying web features. |

Technically you -could- do it (I could probably even make you the cursor), it just wouldn't behave the same and would have a triangle in the upper-left corner of the box as an indicator.

What, you mean the character-sized rectangle that jumps around the screen as you move the mouse? That's not possible in CSS. If you're talking about something else, I have no idea. |

CSS cursors select from the different system cursors a user has on his computer (e.g. crosshairs, "help", caret, etc.). If you're talking about the special picture cursors, the ones with shitty photographs with little white arrows to show where you're actually pointing, you're looking for Comet Cursors.
