This link seems to have the solution to your problem (or a similar one at least).

When I boot up the ol' computer (running Windows XP btw), I get the regular blue screen with the accounts that I can log in under. There are two, one for my family, and one specifically for me.

But when I get to this screen after booting up, a window shoots up with the error:
Windows - System Error
A duplicate name exists on the network.
(OK button)

By pressing the OK button, the error goes away and totally doesn't bother me.

The part that bothers me is that the error is coming up at all. I can't figure out why. Any ideas?
