It was rotoscopped actually. Some scenes were made out of mere references, like winona ryder's fake topless scene.

What I usually do when I want to emulate such effect, is turning the image into B&W (not grayscale!), set it as a layer in multiply mode, and then paint it in the background with a regular brush.

You might want to take a look at the Satndard Template Library. Together with the Boost Library, it is almost a blessing for this stuff.
