It may check for " +" or just "+". If it's not there, it adds " +" to the end. If it is there, it does nothing.

It's possible he added a flag to the users table specifically for this purpose, but unlikely.

Well I don't. Too many people are using it, forcing me to click screenshots instead of just being able to gaze at them and see all the detail in an instant. Not to mention that when they're attached, you can't view them in the thread overview when posting a reply.
