I keep trying to use a star warp in what was originally forest of illusion submap. It works fine... until I try to go back. when i try to warp back, it takes to the middle of Yoshis Island! This happened on one of my previous hacks... but it took me to the middle of Overworld. Any idea what's wrong?

Actually, I have a very similar problem. In one direction the star warp works fine, but if I want to go back it will send me somewhere on the Special World map. From what I can see, the connection path-thing is set up correctly for both directions.

select one star road and click on the pipe icon.make sure that the Exit index from warp star 1 is the destination of warp star 2 and destination of 1 is index of 2

That's not the problem. That was of course the first thing I've checked.

Warp Star 1: Exit index to use: 00: 0000,000C Vanilla Dome
Destination exit index: 0F: 0011,001A Star Road

Warp Star 2: Exit index to use: 0F: 0011,001A Star Road
Destination exit index: 00: 0000,000C Vanilla Dome

Or is it neccessary to have events on the star tiles (actually they have both "N/A", because they do nothing but warp)? I think not.

Or it is neccessary to start a completely new game before it works? I think not, because I set up a few pipes and level tiles before and they all worked right without starting a new game.

I don't think events are the problem. Three of the star tiles in the normal SMW rom have N/A events (and a bunch of places that theoretically should have paths don't have paths, which just confuses me)

I'd reccommend starting a new game. If trying to go through all the levels again is too time-consuming, just drop a goal sphere on the entrance screens. Great for debugging.
